What kind of mismatch causes it?

A difference between what Meta holds about the business and what has been supplied, which sounds small and is not.

Business verification ties a legal entity to the account: name, address, registration details and supporting documents. When something changes on the business side and is not updated with Meta, or when what was submitted does not match a registry Meta checks, the account can be restricted until it reconciles.

The two step verification PIN is the other common cause and is easier to fix. It is set on the phone number and required for certain operations, and if it has been forgotten or set by someone who has since left, it needs resetting before anything else will work.

Where do you actually look?

Business Manager, on the business account rather than on the phone number.

The verification status and any enforcement notice both live there, and between them they usually say which of the two causes applies. A provider dashboard can tell you a number stopped working and when, but it cannot see a verification record, so it cannot tell you which detail is wrong.

If verification is the cause, updating the business details and resubmitting is the path. If the PIN is the cause, it is reset from the WhatsApp Manager settings for that number, and it is worth recording somewhere the business can find it afterwards.
